By registering, I give permission for UIM to photograph or record me and any minor child(ren) included in this registration for ministry-related use (website, social media, print, and promotional materials). I understand no compensation is provided and may opt out by notifying UIM in writing prior to the event. I confirm I am the parent/legal guardian of any minor child(ren) registered and have authority to grant this permission. If I register another adult, I am responsible for advising them that photos or recordings may be taken and that they must notify UIM in writing prior to the event if they do not want their image used.
